I am able to call the SIM card in the RUT240 and trigger the digital output, so I know the phone network and the SIM card are working Ok.
I have tried the SIM card in another phone and can receive and send SMS messages perfectly.

The logs show zero (0) SMS received but I can send SMS via the console/RUT240WEB page and these show in the logs.
SMS utilities are all enabled and Authorization Method is set to "No Authorization".

Thanks for attaching the troubleshoot file it was really helpful!

I see that your device is running firmware version RUT2XX_R_00.01.14.5.  Please try to update the firmware of your device, it is normal that when you purchase a new device it comes with an older version of firmware. You can find the latest version of firmware for your RUT240 at this link: https://wiki.teltonika-networks.com/view/RUT240_Firmware_Downloads

The latest version is RUT2_R_00.07.01.4  you can upgrade following the next instructions:
1. Navigate to the System tab and click the Firmware option. 

2. There you can upgrade to the next available firmware through the one available on the server or the one that you downloaded.

This upgrade has now been completed and the same problem exists.

Ok, So once I waited 1 hour after the upgrade the RUT240 started to read the historical SMS messages and then started to bombard me with SMS responses to those historical messages!
Great news it now works!
